Pre-Production Waiver

I, ______________________, owner and/or authorized representative of ______________________, acknowledge and understand that I must provide the following information to Laundry Boss LLC (“TLB”) prior to the production of my order.

1. Completed Machine List (Excel Spreadsheet to be provided in “Welcome Packet.”)
2. Clear Matterport of Location taken by you, a location manager or arrangements can be made for a professional scan (fees apply).
3. Clear photos of machine model’s front, internal board, and coin mechanism. (Please see our guide on Taking Effective Photos for TLB Service/Installation.)
4. Full disclosure as to whether any machines at your location have had any mechanical replacements, repairs, or are completely refurbished.

I understand that TLB will use the information provided to create a layout of my location to ensure accuracy of machine numbering, and placement of the TLB HUB(s). I also acknowledge and understand that once I have approved said layout, any and all adjustments I wish to make, must be provided to TLB in writing prior to the installation of my system.

I also understand and agree that if installation is delayed beyond 90 days from the date the contract is executed due to my lack of response, communication or action, monthly billing will commence on the ninety-first (91st) day. Should this be a new build, or a circumstance previously disclosed during the quoting process that will prolong installation past the 90-day threshold, billing will commence thirty (30) days post-install as stated in the contract.

THIS WAIVER is made effective as of the date indicated below.
